I sort of thought so at first as well, but If you watch Tom's face when he is receiving the kick, instead of watching the actual kick, his face has quite the amount of shock value.  Watch again and see if you may change your mind.

Regardless, real or fake, I honestly thought it looked unstaged, I loved it mostly because I think Tom Cruise is a big "doucher" yes.
