Does anyone know what Brain has going on in his headphones whilst playing live?

He probably has sensitive ears or something, a lot of drummers wear them to avoid going deaf. Including Keith Moon

In that case why not wear smaller earplugs? I thought it was maybe a click track to help him stay in time or the bass / guitars coming from one of the monitors.

They're monitors.

You can tell in 02 he was having problems with his monitor system.  All of those players onstage, and pyro going off I'm sure it was hard to hear back there.  Look at Rio in 01 at the start of YCBM for example.  You can tell he couldn't hear a damn thing.

Wearing headphones as a drummer is a wise decision in a big band like that.

He probably got a metronome in the headset, many drummers do

i also think bcoz now in '06 they use synth drums aswell. he just has a feed of his own drums so he doesnt get confused. alot of drummers do that in bands that also have snth drums
