I found this cool old NBC news report on Axl on YouTube.  It talks about him getting in a fight with his neighbor and he says "I live next door to a psycho".  Its pretty entertaining.

yeah apparently thats what inspired the title at least and the "fuck you bitch" part.

The rest of the song sounds like Axls take on fame and the changes he underwent as a result of becoming famous to me

The record company wanted GnR to open the album with that song because they thought it captured the intensity of WTTJ as an opening track....   I love the song... its very under rated.   ok

I love Right Next Door to Hell, but as far as the intensity of Jungle, they should have opened with Perfect Crime. 

But yeah, to me it sounds like his neighbor only inspired the title of the song, and the angry vibe, but the true meaning of the song seems to be pretty deep to me.
